Summary
Ecnoglutide (also known as XW003) is a long-acting, biased GLP-1 receptor agonist developed by Sciwind Pharmaceuticals. Unlike conventional GLP-1 agonists, ecnoglutide is designed to preferentially activate G-protein signalling pathways over β-arrestin recruitment, which may offer improved efficacy and tolerability. Phase 3 trial results published in 2025 demonstrated significant weight loss in adults with overweight or obesity. It is not currently licensed in the UK.
Mechanism
Ecnoglutide is a biased GLP-1 receptor agonist. It preferentially activates the G-protein signalling pathway (cAMP/PKA) at the GLP-1 receptor while showing reduced β-arrestin recruitment compared to unbiased agonists like semaglutide. This biased signalling profile is hypothesised to sustain receptor activity (less desensitisation/internalisation) and may reduce gastrointestinal side effects. The net effect is enhanced insulin secretion in a glucose-dependent manner, suppression of glucagon, delayed gastric emptying, and increased satiety — consistent with GLP-1 receptor pharmacology.
Evidence base
A Phase 3 randomised, double-blind, placebo-controlled, multicentre trial published in The Lancet Diabetes & Endocrinology (2025) demonstrated statistically significant weight loss with ecnoglutide in adults with overweight or obesity. This is the first Phase 3 readout for the compound. Evidence grade: Moderate. The trial supports ecnoglutide's efficacy as a GLP-1 RA, but head-to-head comparisons with approved agents and long-term cardiovascular outcomes data are not yet available.
Protocols
In Phase 3 trials, ecnoglutide was administered as a once-weekly subcutaneous injection with a dose-titration schedule. Specific dose levels have not been prominently disclosed in publicly available topline results. As an investigational compound, no approved dosing guidelines exist.
UK legal status
Ecnoglutide is not licensed by the MHRA and is not approved as a medicine in the UK. It is an investigational peptide in the grey area — legal for research purposes but not available as an approved therapeutic. The MHRA has not evaluated ecnoglutide for safety, quality, or efficacy.
