Mechanism

Petrelintide is a long-acting amylin analogue. Amylin (islet amyloid polypeptide, IAPP) is co-secreted with insulin from pancreatic beta cells and acts on amylin receptors (comprising the calcitonin receptor complexed with RAMP proteins) in the brain. Activation produces reduced food intake (satiety), delayed gastric emptying, and suppression of postprandial glucagon secretion. This mechanism is distinct from and complementary to GLP-1 receptor agonism, making amylin analogues attractive both as monotherapy and in combination with incretin agents.

Evidence base

Positive Phase 2 results for petrelintide were announced by Zealand Pharma in 2025, demonstrating statistically significant weight loss compared to placebo in adults with overweight or obesity. The compound was reported as generally well tolerated. Evidence grade: Limited. These are preliminary Phase 2 topline results — full peer-reviewed publication is pending, Phase 3 trials have not commenced, and long-term safety data are unavailable.

Protocols

In Phase 2 trials, petrelintide was administered as a once-weekly subcutaneous injection. Specific dose levels have not been prominently disclosed in publicly available topline results. As an investigational compound, no approved dosing guidelines exist.
